When did your symptoms start?

Have you had a UTI in the past 6 months?

Have you noticed blood in your urine?

Are you pregnant or breastfeeding?

Do you have any of the following conditions?

Do you have a fever, flank pain, nausea, vomiting, or back pain?

These may indicate a kidney infection

Do you have any known allergies, especially to nitrofurantoin or other antibiotics?

Are you taking any other medicines?

Prescribed, over-the-counter, or herbal

Do you understand the following?

• Nitrofurantoin should be taken with food and plenty of fluids
• You should seek medical help if your symptoms worsen, you develop fever or back pain, or your symptoms do not improve after 3 days of treatment
• This treatment is not suitable for preventing recurrent UTIs or for kidney infections
